I assist clients to knock down barriers and provide resources and work with other providers to improve clients overall mental and physical state.
I have an educational background with classroom experiences, and mental health. I’ve also had extensive experiences working with diverse populations from low income to a variety of individuals with different backgrounds and ethnicities.
Talk with your current Canvas Health provider about any difficulties you or a loved are facing. They can then make a referral for Care Coordination to assist you and your family.
