Care Coordinator
I believe that working together in teams and or in collaboration gives a higher success in achieving goals.
I am familiar with the CCBHC model. My previous roles/experience was a case manager, ARMHS worker and being a Care Coordinator for our ACT team.
Talk with your current Canvas Health provider about any difficulties you or a loved are facing. They can then make a referral for Care Coordination to assist you and your family.
