I have worked in Human Services for more than 7 years with a primary focus on providing advocacy to individuals experiencing domestic violence, sexual violence, and trafficking, in addition to experience as a county Social Worker.
Talk with your current Canvas Health provider about any difficulties you or a loved are facing. They can then make a referral for Care Coordination to assist you and your family.
