Canvas Health’s supportive housing services help people live more stable, productive lives. Since 1990, the Canvas Health housing program has assisted adults with mental illness and substance use disorders in obtaining and maintaining safe, affordable, and stable housing.
Canvas Health does not provide housing for community members.
Canvas Health’s Service Coordination is a program that works in conjunction with housing services to provide onsite social workers in buildings occupied by seniors. They offer information, referral and general housing support.
Service Coordinators work in conjunction with property management companies that provide housing for seniors and disabled individuals. The goal of service coordination is to help residents maintain stable housing and to remain living independently in the community. Service Coordinators provide education and support resources for tenants to help identify issues that could jeopardize their leases or might otherwise impact their housing in a negative way.
SHARE (Sober Housing and Recovery Environment): Canvas Health provides permanent subsidized housing and supportive services for substance dependent, homeless, single parents and their children in Forest Lake. The goal is to help parents and their children become self sufficient and contributing members to the community. Case managers provide advocacy to help parents address substance abuse problems that have often resulted in their involvement in the legal system and child protection services.
