Voluntary or Self-Referred Assessments

Canvas Health offers Substance Use Comprehensive Assessments to help individuals understand if and how their substance use may be affecting their health, relationships, or mental health. People may seek assessments if they notice increased tolerance, using subtances to cope with stress or emotions, or if they experience an impact on their sleep or mood.

A person might seek an assessment to help manage the use of or addiction to:

  • Alcohol
  • Marijuana
  • Cocaine
  • Heroin
  • Methamphetamines
  • Opioids
  • Other drugs

Canvas Health also can complete assessments to fulfill requests from an employer or as requirements for retention of a professional licensure.

After the assessment is complete, the assessor will give you personalized recommendations that may include outpatient support, mental health services, harm reduction strategies, and/or community resources.

Court-Ordered Assessments

Substance Use Comprehensive Assessments are also available to individuals who have been ordered by a court or legal jurisdiction to receive one. Our evaluations meet legal, court, and probation requirements in the state of Minnesota.

Substance Use Comprehensive Assessments provide a judge with an objective assessment of your situation regarding alcohol or drug use.

If you are involved with a legal case concerning DWI (Driving while Intoxicated), DUI (Driving Under the Influence), public intoxication, possession of drugs or drug paraphernalia, or serious crimes like assault and battery or disorderly conduct, you may be required by a judge or court to undergo an evaluation related to your use of alcohol or drugs.

Substance Use Assessments are also commonly ordered in civil cases involving child custody, divorce, or license reinstatement. It is used as evidence in a court proceeding to help judges understand how your alcohol or drug use may have affected the circumstances of your legal situation. It may contribute to the adjudication of the case, as well as any decisions related to consequences of the case.

What is a Substance Use Comprehensive Assessment?

A Substance Use Comprehensive Assessment (previously known as a Rule 25 Assessment) is a fact-based account of your history with substance use.

Assessments are also used in developing care management plans for adults, adolescents, and their families.

Substance use assessments contain confidential information. This information is released to third parties such as courts only after you have provided your consent.

Are Assessments Conducted By Licensed Clinicians?

Yes. All members of our staff are licensed alcohol and drug counselors, and are qualified to evaluate and help manage substance use problems. Our assessments are recognized and accepted by all counties in the State of Minnesota.

What’s the Process for Getting an Assessment and How Long Does It Take?

During an assessment, one of our licensed alcohol and drug counselors will use interviews and diagnostic screenings to collect personal biological, social, psychological and historical data from you. Typically, the interview component of a Substance Use Comprehensive Assessment will last between forty-five minutes to one hour.

The substance use assessor uses the data to make formal recommendations about treatment. The information you share is confidential. Nothing is shared with any outside parties without your consent.

Do Substance Use Comprehensive Assessments Involve a Physical Examination?

There are no physically invasive procedures involved in a Substance Use Comprehensive Assessment. Occasionally, the courts, another legal entity or one of our assessors may request a urine sample for testing.

Substance Use Comprehensive Assessments must be face-to-face, so they cannot be performed over the telephone. They can, however, be provided via telehealth as an alternative to an in-person meeting.

What Do I Need to Prepare Before My Assessment?

In order to complete a Substance Use Comprehensive Assessment, your clinician will need access to certain documents and information from your life story. This includes:

  • Medical records, including all medications and prescriptions you regularly take for your physical and mental health. If you have ever consulted with a psychiatrist, the clinician will need to know about any diagnoses, medications, and other treatments.
  • Legal records, including detailed accounts of any arrests and their outcomes, as well as any relevant civil proceedings.
  • Biographical records, including information pertaining to marriage, divorce, employment, and education. Information about your involvement in your community may also be requested.
